Get a memory assessment for a worried relative

Raise memory concerns gently with a relative, gather useful examples and help them through a GP visit and referral to a memory assessment service.

Use this guide if a parent, partner or other relative is becoming more forgetful, confused or different in ways that worry you, and you want them checked by the NHS. Have a notebook for examples and, if possible, the relative's agreement to involve their GP.

Step by step

  1. Keep a short record of changes

    Over two to three weeks, note specific examples with dates: repeated questions, missed bills or appointments, getting lost on familiar routes, trouble with words, changes in mood or personality, or problems managing medicines. Include how long the changes have been happening. Many things can cause memory problems, including infections, low mood, medicine side effects and vitamin or thyroid problems, which is why a check-up matters.
